The list of things it lacks ranges from guild tabards to player-versus-player combat. Perhaps the most significant lack is solo content: Most people will find the dungeons too tough to go it alone, so you'll have to pull together a group. The game provides a nice interface for finding other people to adventure with, but that won't mollify people who like the option to go solo.
For many players, that's going to be a downside, plain and simple. But for those looking to re-create the pencil-and-dice Dungeons & Dragons experience in an online multiplayer game, it's a feature. No game, including Neverwinter Nights, comes as close to feeling like an actual old-school D&D module running on your computer screen.
